To complete a member search, you must enter data in ALL of the required(*) fields.
Manage Generic Web Accounts
Step 1 - Enter the required member information which can be found on your ID card. The member number is your 9-digit member number and your 2-digit member identifier from your ID card (such as 01 or 02).
Step 2 - After submitting your personal member information you will be brought to a screen to create a User ID and password.
Step 3 - Once your account is successfully created, you will be sent to the login screen to login with your new account information.
If there is an issue matching your personal member information you will be directed to call a Dean Health Plan Customer Service Representative to assist you.
